Thank you for being a valued cardholder and participating in the My Card Extras Plus Program.
The My Card Extras Plus program was discontinued on December 31, 2018. As of December 31, 2018, you will no longer be able to utilize the benefits of the program. You will not be billed for the My Card Extras Plus program on your account after this date.
You will have an additional 90 days following this date to submit a reimbursement request for benefits used while you were an active member of the program. All requests must be postmarked no later than March 31, 2019 in order to be reviewed. To obtain a form for reimbursement, please click on "Reimbursement Forms" below. Forms can be printed and mailed to the address located on the form.
